{"id":345,"date":"2022-10-31T11:04:21","date_gmt":"2022-10-31T11:04:21","guid":{"rendered":"https:\/\/careandrepair.org.uk\/cy\/2023\/01\/24\/bridgend-care-repair-celebrates-supporting-10000-patients-31-10-2022\/"},"modified":"2023-02-23T11:49:40","modified_gmt":"2023-02-23T11:49:40","slug":"bridgend-care-repair-celebrates-supporting-10000-patients-31-10-2022","status":"publish","type":"post","link":"https:\/\/careandrepair.org.uk\/cy\/bridgend-care-repair-celebrates-supporting-10000-patients-31-10-2022\/","title":{"rendered":"Gofal a Thrwsio Pen-y-bont ar Ogwr yn dathlu cefnogi 10,000 o gleifion"},"content":{"rendered":"<h3><strong>Mae Gofal a Thrwsio Sir Pen-y-bont ar Ogwr wedi cyrraedd y garreg filltir hynod o dderbyn ei 10,000fed atgyfeiriad, wrth iddo helpu rhyddhau cleifion yn gyflym ac yn ddiogel o Ysbyty Tywysoges Cymru ym Mhen-y-bont ar Ogwr.<\/strong><\/h3>\n<p>Mae gwasanaeth Ysbyty i Gartref yr elusen yn cynnal ymyriadau diogelwch cartref, tebyg i f\u00e2n addasiadau, atgyweiriadau a darparu gwresogi ychwanegol, i helpu hwyluso rhyddhau pobl h\u0177n yn brydlon ac yn ddiogel o\u2019r ysbyty.<\/p>\n<p>Gan weithio mewn partneriaeth gyda staff ysbyty mae\u2019r gwasanaeth yn derbyn dros 1,500 atgyfeiriad y flwyddyn ac yn helpu i ostwng amser aros mewn ysbytai drwy arbed tua 8,000 o ddyddiau gwely bob blwyddyn.<\/p>\n<p>I ddathlu ei lwyddiant, cynhaliodd y gwasanaeth Ysbyty i Gartref ddigwyddiad yn Ysbyty Tywysoges Cymru a fynychwyd gan Huw Irranca-Davies AS, Sarah Murphy AS, a Sarah James Dirprwy Gyfarwyddwr Gweithrediadau Bwrdd Iechyd Prifysgol Cwm Taf.<\/p>\n<p>Dywedodd Rena Sweeney, Cyfarwyddwr Asiantaeth Gofal a Thrwsio Sir Pen-y-bont ar Ogwr: \u201cRydym wrth ein bodd i gyrraedd y garreg filltir bwysig yma, ond mae\u2019n rhaid i mi dalu teyrnged i holl waith caled ac ymroddiad staff y gwasanaeth. Mae\u2019r gwasanaeth wedi cyflawni cymaint ers ei gyflwyno yn 2014 a chafodd y model a sefydlwyd ym Mhen-y-bont ar Ogwr ei atgynhyrchu gan Care &#038; Repair Cymru mewn ysbytai ar draws Cymru\u201d.<\/p>\n<p>Meddai Meinir Woodgates, Rheolwr Gwasanaeth Ysbyty i Gartref: \u201cMae cyflymder a nifer y gwelliannau cartref a gwblhawyd wedi golygu fod cleifion a atgyfeiriwyd i\u2019r gwasanaeth yn aros chwe diwrnod yn llai yn yr ysbyty ar gyfartaledd. Mae\u2019r gwasanaeth hefyd yn helpu i ostwng nifer y cleifion sy\u2019n gorfod dychwelyd i ysbyty.\u201d<\/p>\n<p>Ychwanegodd Chris Jones, Prif Weithredwr Care &#038; Repair Cymru: \u201cMae Ysbyty i Gartref yn datrys problem. Mae\u2019r gwasanaeth yn gweithio gyda staff ysbyty, cleifion a\u2019u teuluoedd i ddynodi a datrys problemau tai a fyddai fel arall yn atal pobl rhag cael eu rhyddhau o ysbyty.<\/p>\n<p>\u201cMae\u2019n glod i ymroddiad staff Gofal a Thrwsio Sir Pen-y-bont ar Ogwr fod y gwasanaeth Ysbyty i Gartref wedi tyfu\u2019n gyflym o hadau bach i wasanaeth sylweddol sydd ar gael ledled Cymru. Ni fyddai\u2019r twf hwn wedi bod yn bosibl heb reddf cydweithio staff rheng flaen y GIG, yn ogystal ag uwch gydweithwyr ar draws gwasanaethau iechyd, a sylweddolodd botensial y cynnig.\u201d<\/p>\n<p>Yn genedlaethol, caiff y gwasanaeth bellach ei alw yn Ysbyty i Gartref Iachach ac mae\u2019n gweithredu allan o 17 ysbyty.
