Dyna pam fod <a href=\"https:\/\/www.gov.wales\/home-first\">canllaw newydd Llywodraeth Cymru <\/a>yn gosod ymagwedd \u2018gartref yn gyntaf\u2019 i gefnogi pobl ar \u00f4l bod mewn ysbyty.<\/strong><\/p>\n<p>Unwaith y bydd triniaeth feddygol yn yr ysbyty ar ben, yn aml gartref yw\u2019r lle gorau i wella. Gallai gartref fod yn gartref gofal, eich cartref eich hun neu d\u0177 cyfaill neu berthynas.<\/p>\n<p>Mae meddwl am ddychwelyd gartref cyn gynted \u00e2\u2019ch bod yn mynd i\u2019r ysbyty yn arbennig o bwysig ar gyfer pobl h\u0177n. Gall aros mewn ysbyty pan ydych yn ddigon da i fynd gartref arwain at ddatgyflyru, sydd pan mae eich iechyd corfforol a meddwl yn gwaethygu oherwydd cyfnodau hir heb fod ar eich traed ac mewn gwely ysbyty.<\/p>\n<p>Os ydych yn aros mewn ysbyty am gyfnod hir, mae\u2019n bwysig dal i godi a gwisgo os gallwch wneud hynny a defnyddio pethau na fyddai gennych gartref fel arfer fel eich sbectol, cymorth clyw, oriawr a dyddiadur i atal datgyflyru.<\/p>\n<p><strong>Ysbyty i Gartref Iachach<\/strong><\/p>\n<p>Mae Gofal a Thrwsio yn deall y gallai fod yn her i fynd gartref yn dilyn cyfnod mewn ysbyty. Dyna pam fod ein <a href=\"https:\/\/careandrepair.org.uk\/projects\/hospital-to-a-healthier-home\/\">gwasanaeth Ysbyty i Gartref Iachach<\/a> yn gweithio gyda chleifion sy\u2019n ddigon da i adael ond na all wneud hynny oherwydd nad yw eu cartref yn ddiogel neu\u2019n addas ar gyfer eu hadferiad. Gallai hynny olygu eich bod angen gwaith addasu fel canllawiau i\u2019ch helpu i fynd o amgylch y t\u0177 ar \u00f4l syrthio neu gael str\u00f4c, a gallai eich cartref fod angen gwaith trwsio i\u2019ch atal rhag mynd yn wael eto oherwydd pethau fel lleithder a llwydni.<\/p>\n<p>Yn aml bydd meddygon a nyrsys yn atgyfeirio cleifion atom gan wybod y gallent fod angen peth gwaith addasu neu drwsio i\u2019w cartrefi, ond gallwch hefyd ddweud wrth staff ward amdanom ac yr hoffech ein cymorth. Gallwn drefnu y gwaith addasu cyn i chi ddod gartref ac yna alw heibio i wirio sut ydych ac os ydych angen unrhyw beth arall. Gallai hyn fod yn:<\/p>\n<ul>\n<li>Gwiriad budd-daliadau i wneud yn si\u0175r eich bod yn hawlio yr hyn sydd ar gael i chi<\/li>\n<li>Gwaith addasu ychwanegol os ydych yn ei chael yn anodd symud o gwmpas<\/li>\n<li>Gwybodaeth a chyngor ymarferol ar sut i atal syrthio yn y cartref<\/li>\n<li>Gwybodaeth ar gontractwyr a chymorth lleol<\/li>\n<\/ul>\n<p>Ers 2019 mae Ysbyty i Gartref Iachach wedi cefnogi dros 21,400 o gleifion i adael yr ysbyty yn gyflymach i gartrefi mwy diogel a chyfleus. Amcangyfrifwn ein bod wedi arbed cyfanswm o tua 131,000 o ddyddiau gwely i GIG Cymru. Mae meddwl \u2018gartref yn gyntaf\u2019 yn helpu eraill sy\u2019n aros am wely ac yn gwella llif cyffredinol pobl drwy\u2019r ysbyty ar gyfer gofal wedi ei gynllunio a heb ei
