{"id":1701,"date":"2024-01-15T10:49:22","date_gmt":"2024-01-15T10:49:22","guid":{"rendered":"https:\/\/careandrepair.org.uk\/cy\/welsh-government-recognise-the-importance-of-home-adaptations-in-facilitating-safe-hospital-discharge\/"},"modified":"2024-01-19T17:01:47","modified_gmt":"2024-01-19T17:01:47","slug":"welsh-government-recognise-the-importance-of-home-adaptations-in-facilitating-safe-hospital-discharge","status":"publish","type":"post","link":"https:\/\/careandrepair.org.uk\/cy\/welsh-government-recognise-the-importance-of-home-adaptations-in-facilitating-safe-hospital-discharge\/","title":{"rendered":"Llywodraeth Cymru yn cydnabod pwysigrwydd addasiadau cartref wrth hwyluso rhyddhau diogel o ysbyty."},"content":{"rendered":"<h3><strong>Mae Llywodraeth Cymru wedi cyhoeddi ei chanllawiau diweddaraf ar <a href=\"https:\/\/www.gov.wales\/written-statement-update-hospital-discharge-and-pathways-care-delay-reporting-framework\">ryddhau o\u2019r ysbyty<\/a>, sy\u2019n disodli canllawiau a gyhoeddwyd yn ystod pandemig Covid-19.<\/strong><\/h3>\n<p>Anelwyd y canllawiau at staff ysbyty i gefnogi cydlynu effeithiol ar gynllunio rhyddhau o ysbyty, yn arbennig ar gyfer cleifion a all fod angen mwy o ofal unwaith y cawsant eu rhyddhau o ysbyty.<\/p>\n<p>Rydym yn falch i weld fod y canllawiau yn cydnabod r\u00f4l bwysig offer ac addasiadau cartref wrth helpu cleifion i adael ysbyty a byw\u2019n ddiogel ac annibynnol yn eu cartrefi eu hunain.<\/p>\n<p>Mae\u2019n dweud, \u201cRhaid cynllunio addasiadau sy\u2019n galluogi ac yn helpu i ryddhau cleifion &#8230; mor fuan \u00e2 phosibl ar \u00f4l derbyn claf fel rhan o\u2019r cynllun adfer.\u201d<\/p>\n<p>Bu Gofal a Thrwsio yn galw ers amser maith am i amgylchedd y cartref gael ei ystyried yn gynnar wrth gynllunio rhyddhau o ysbyty. Mae ein Gwasanaeth Ysbyty i Gartref Iachach wedi gweithredu ar draws Cymru ers 2019, gan ddarparu addasiadau a gwelliannau cartref i gleifion sy\u2019n ddigon da i fynd adre ond na all adael yr ysbyty oherwydd problem gyda\u2019u cartref neu amgylchedd.<\/p>\n<p>Y llynedd fe wnaeth y gwasanaeth arbed bron 26,000 o ddyddiau gwely i NHS Cymru drwy gwblhau dros 7,200 o addasiadau a gwelliannau cartref cyflym yn werth dros \u00a31.7 miliwn ar gyfer dros 4,000 o gleifion. Roedd y gweithiau hyn yn hanfodol ar gyfer rhyddhau diogel o ysbyty ac atal pobl rhag gorfod aros mewn ysbyty am fwy o amser nag oedd yn hanfodol. Yn fwy eang yn y gymuned, y llynedd fe wnaeth Gofal a Thrwsio helpu dros 62,000 o bobl h\u0177n drwy gwblhau dros 65,000 o swyddi yn y cartref i gadw pobl h\u0177n yn ddiogel yn eu cartrefi eu hunain drwy atal syrthio a damweiniau yn y cartref ac atal derbyniadau ysbyty.<\/p>\n<p>Mae <a href=\"https:\/\/careandrepair.org.uk\/projects\/hospital-to-a-healthier-home\/\">gwasanaeth Ysbyty i Gartref Iachach<\/a>\u00a0Care &amp; Repair Cymru yn gydnaws gyda pholisi Chwe Nod ar gyfer Gofal Brys ac Argyfwng Llywodraeth Cymru, a ffocws y Gweinidog i feithrin capasiti cymunedol i gael cleifion allan o ysbyty \u2018ymhellach a chyflymach\u2019. Gobeithiwn y bydd fersiynau a gyhoeddir yn y dyfodol o\u2019r canllawiau rhyddhau o ysbyty yn cydnabod r\u00f4l ganolog Gofal a Thrwsio wrth gyflenwi addasiadau cartref i hwyluso rhyddhau diogel o
