Mae ei iechyd yn dechrau gwaethygu, ond nid yw eisiau symud bant o\u2019r cartref a\u2019r gymuned y mae\u2019n eu caru. Dyna lle camodd Gofal a Thrwsio i mewn.<\/h3>\n<p>\u201cMae Gofal a Thrwsio wedi fy helpu oherwydd pan oeddwn i\u2019n dod allan o\u2019r bath roeddwn yn gorfod cael step ac roedd gen i ofn mawr y byddwn yn colli fy malans ac yn syrthio\u201d, meddai Dai.<\/p>\n<p>Roedd Dai wedi prynu ei gartref am ddim ond \u00a3350 yn 1965 ac mae\u2019n falch ei fod yn dal i fyw yno. Anfonodd Gofal a Thrwsio y gweithiwr achos Heulwen i weld sut gallem helpu.<\/p>\n<p>Dywedodd Heulwen, \u201cFel ateb tymor byr fe wnaethom roi canllawiau bach dros y bath i\u2019w helpu i fynd mewn ac allan. Ond roedd yn dal yn anodd, felly fe wnes atgyfeiriad i therapi galwedigaethol gydag argymhellion ar gyfer grant cyfleusterau i\u2019r anabl ar gyfer addasiadau i\u2019r ystafell ymolchi.\u201d<\/p>\n<p>Gosodwyd cawod ystafell wlyb newydd ar gyfer Dai, gan olygu y medrai ymolchi heb ofni syrthio.<\/p>\n<p>Sylwodd Heulwen hefyd nad oedd Dai yn clywed gystal ag yr oedd. Dywedodd, \u201cMae Mr Meredith wedi colli peth o\u2019i glyw, felly roedd clywed cloch y drws yn un o\u2019r pethau oedd yn anodd iddo. Felly fe wnes archebu cloch drws gyda goleuadau yn fflachio arni. Os na all glywed cloch y drws, gall weld y goleuadau\u2019n fflachio.<\/p>\n<p>Trefnwyd hefyd i i roi canllawiau yn yr ardd ac wrth ymyl y drysau i gartref Dai. Mae hyn wedi galluogi Dai i fynd o amgylch yn ddiogel ac yn rhwydd.<\/p>\n<p>Mae gan wasanaeth Ymdopi\u2019n Well Gofal a Tthrwsio weithwyr achos neilltuol, fel Heulwen, sy\u2019n cefnogi pobl gyda
