{"id":1083,"date":"2022-11-27T11:46:45","date_gmt":"2022-11-27T11:46:45","guid":{"rendered":"https:\/\/careandrepair.org.uk\/cy\/?p=1083"},"modified":"2023-02-23T11:49:52","modified_gmt":"2023-02-23T11:49:52","slug":"new-report-10000-patients-helped-through-hospital-to-a-healthier-home","status":"publish","type":"post","link":"https:\/\/careandrepair.org.uk\/cy\/new-report-10000-patients-helped-through-hospital-to-a-healthier-home\/","title":{"rendered":"Adroddiad Newydd: Ysbyty i Gartref Iachach"},"content":{"rendered":"<p><strong>Adroddiad Newydd: Ysbyty i Gartref Iachach<\/strong><\/p>\n<p><strong>Mae Gofal a Thrwsio wedi cyhoeddi adroddiad gwerthuso newydd yn tynnu sylw at lwyddiannau prosiect Ysbyty i Gartref Iachach dros y tair blynedd ddiwethaf.<\/strong><\/p>\n<p>Mae Ysbyty i Gartref Iachach ar gael mewn 17 ysbyty ar draws Cymru ac mae\u2019n gweithio i sicrhau y gall cleifion gael eu rhyddhau\u2019n gyflym a dychwelyd i gartref diogel, cynnes a chyfleus.<\/p>\n<p>Dengys yr adroddiad newydd fod y prosiect wedi arbed dros 62,000 o ddyddiau gwely i GIG Cymru dros y tair blynedd ddiwethaf. Dywedir fod yr ystadegyn anhygoel hwn oherwydd cyflymder a nifer yr addasiadau y gall y gwasanaeth eu cwblhau, gan olygu fod cleifion a atgyfeirir at y prosiect yn aros yn yr ysbyty am chwe diwrnod yn llai ar gyfartaledd. Mae\u2019r adroddiad hefyd yn rhoi sylw i r\u00f4l y gwasanaeth mewn gostwng nifer y cleifion sy\u2019n gorfod mynd yn \u00f4l i ysbyty. Mae gan gleifion a gafodd eu helpu gan y prosiect 5.7% o gyfle i byddant yn gorfod dychwelyd i ysbyty o gymharu gyda chyfartaledd bwrdd iechyd o tua 12-15%.<\/p>\n<p><strong>Dywedodd Faye Patton, Rheolwr Prosiect Ysbyty i Gartref Iachach<\/strong> \u201cMae\u2019r adroddiad newydd hwn yn dangos r\u00f4l hanfodol y prosiect mewn ysbytai yng Nghymru. Mae\u2019n wych y cafodd dros 10,000 o gleifion eu hatgyfeirio atom yn nhair blynedd cyntaf bod yn brosiect i Gymru gyfan ac mae\u2019n dangos pwysigrwydd cydweithio a phartneriaeth yn y sector iechyd a pham fod yn rhaid i wasanaethau fel hyn barhau i dderbyn cyllid.\u201d<\/p>\n<p>Mae gweithwyr achos Ysbyty i Gartref Iachach yn gweithio\u2019n uniongyrchol gyda staff ysbyty i ddynodi cleifion h\u0177n sydd \u00e2 phroblemau tai a all ohirio eu dychweliad adre. Mae timau Gofal a Thrwsio wedyn yn gweithio gyda chleifion a\u2019u teuluoedd i gynnal y gwelliannau cartref sydd eu hangen i alluogi cleifion i gael eu rhyddhau\u2019n gyflym ac yn ddiogel.<\/p>\n<p><strong>Mae Eleri Evans, Pennaeth Nyrsio Ysbyty Gwynedd, yn gweld manteision clir y prosiect:<\/strong><\/p>\n<p>\u201cYr hyn rydyn ni eisiau yw ychydig bach o adnoddau ychwanegol i gefnogi pobl yn eu cartrefi eu hunain, a gallai hynny fod yn rhywbeth mor syml \u00e2 chanllaw. Mae mor syml \u00e2 hynny. Yn hanesyddol, byddai\u2019n rhaid i ni wneud d atgyfeiriad gwaith cymdeithasol. Fe fyddai\u2019n rhaid i ni aros i weithiwr cymdeithasol gael ei ddyrannu. Felly o\u2019n safbwynt ni, mae Ysbyty i Gartref Iachach yn werthfawr tu hwnt.\u201d<\/p>\n<p><strong>Dywedodd Tracy Daniel, Arweinydd T\u00eem Orthopaedeg ysbyty Tywysoges Cymru:<\/strong><\/p>\n<p>\u201cMae safon rhyddhau yn llawer gwell ers i ni gael Gofal a Thrwsio oherwydd fod llawer mwy o wiriadau nag y byddem wedi eu gwneud a gweithredu arnynt o\u2019r blaen. Os oes gennych bobl gyda phethau fel problemau ar y frest sy\u2019n byw mewn cartref llaith, gall hynny gael ei newid nawr, gan olygu nad ydynt yn gorfod mynd yn \u00f4l i ysbyty yn y dyfodol oherwydd cyflwr y t\u0177. Felly mae\u2019n sgil-effaith ataliol\u201d.<\/p>\n<p>Mae\u2019r gwasanaeth yn gwella llif cleifion ac yn gostwng nifer yr achosion sy\u2019n gorfod mynd yn \u00f4l i ysbyty. Amcangyfrifir fod hynny\u2019n arbed 25,000 o ddyddiau gwely bob blwyddyn i\u2019r GIG yng Nghymru.<\/p>\n<p><a
